Your 1099R can be accessed via a secure online download. Please follow the steps listed below. This can be from any computer connected to the internet.

1. Open your web browser (Internet Explorer, Firefox)
2. Make sure to turn off the pop up blocker
3. Type in the following web address - www.downloadmyform.com/fmptf
4. Click in the empty box and enter your Social Security Number, dashes are not necessary.
5. Tab to the next empty box and next to “Name” type your first and last name. You may also need to insert your middle initial between your first and last names.
6. Click Sign in
7. Once the next page opens you will click on the "get my access code" link
8. This will open a new browser page with the access code on it
9. Write down the access code and close the new browser page
10. Type the access code in the box provided and click the small box under the access code consenting to get your 1099R electronically
11. Click Continue
12. Click on the small copy of the 1099R on the left side of the page
13. A PDF version of your 1099R will pop up, at this point you can print the 1099R to a printer or you can save a copy to be printed later
14. Once you have printed a copy of the 1099R simply sign out of the website by clicking on the Sign out button near the top right hand corner of the page.

Please remember this information is confidential so do not leave your computer with your information on the screen. Also if you print your 1099R to a public printer make sure you go immediately to that printer and pick up the copy. You will have until around January 23rd to access your form online.
